Your First Visit to Elite IVF Solutions
After making your first appointment with Elite IVF Solutions, you will receive a welcome packet via email. This packet contains:
- Information confirming your appointment/consultation
- Directions to our state-of-the-art center
- Important medical forms* that will need to be completed prior to your first appointment/consultation:
- Patient Demographics
- Female History (if applicable)
- Male History (if applicable)
- Medical Record Release
* We request these items be mailed, emailed or faxed and received 1 week prior to your scheduled appointment.
On the Day of Your Appointment
We ask that you arrive 15 minutes before your scheduled appointment time and complete and bring with you the following items:
- Insurance card and insurance coverage information
- Driver's license/photo ID
- Completed forms if you were not able to submit these prior to the scheduled appointment
- Insurance co-payment or payment in full
We accept: Cash, personal check, major bank cards.
Much of your first appointment is consultation after reviewing past medical records. It is important for us to have this information before your first visit. We ask that you and your partner complete and fax to us your medical record release form at least 1 week before your appointment/consultation so that we may obtain your medical history.
You may also bring your medical records/information to your first appointment if you cannot obtain them beforehand.
During Your Consultation with Dr. Lindheim
As you meet with Dr. Lindheim, you and your partner will have the opportunity to:
- Ask any questions that you may have
- Address any issues that might be of worry
Fertility concerns are best resolved in consultation with both you and your partner. It might be recommended that you and your partner have some additional medical testing to help identify the cause of your difficulty getting pregnant.
Particular care will be taken so that only the appropriate tests relevant to you and your partner are conducted, saving both time and expense. If this is the case, we can schedule these during your first visit.
After you have completed your initial consultation with Dr. Lindheim, you will then meet on that day with our dedicated financial counselor to discuss insurance benefit and possible payment options/solutions tailored to your treatment plan. We take pride in working with every patient and couple on finding the best financial solution to complement their treatment plan.
